Outreach Service referral process

The school decide to contact the Outreach Service either through recommendation from a Physio, OT, Ed psych or the SENCO for a statement child who has a physical impairment.

   
       
    School ask parents/carers if they are happy for the referral to be made.    
       
   

School refer pupil* to the Claremont Outreach Service for Curriculum, ICT or Physical support, including support with PE, carrying out Risk assessments & Manual Handling advice

School have to look into other support services & look at in school support they can offer.

       

 

 

Claremont Outreach service considers the referral.

 

Claremont Outreach Service will recommend other support services that are accessible

       
   

Claremont Outreach Service will contact the school within 2 weeks to arrange an appointment for an observation or assessment

   
 

 

*Curriculum support

We can agree to support the pupil in school or at sessions at Claremont.

A service level agreement will be signed between Claremont and the Mainstream school

 

*ICT Support

The Mainstream school will sign a service level agreement before the assessment is carried out.

 

 

 

*Physical support

We can agree to support the pupil in school or at sessions at Claremont. A service level agreement will be signed between Claremont and the Mainstream school
